Insurance Information
We work with all PPO or indemnity insurance plans. We will let you know what your estimated out-of-pocket share is for each visit, and we will let you know about limitations on your policy such as frequency of services, applicable deductibles, and annual maximums.
It is important that you keep us apprised of any changes in your dental insurance plan before you come to see us. We may be considered out-of-network providers for some PPO plans, and in-network providers for others.

Moeiz Koshki, DDS
Dr. Moeiz Koshki is a highly experienced general dentist who graduated from Georgetown University School of Dentistry in 1989. After years of practice in the Metropolitan D.C./Northern Virginia area, he relocated to Southern California in 2005 to establish Santa Monica Dental Arts. Trained and licensed in all aspects of restorative and cosmetic dentistry, Dr. Koshki provides comprehensive care to patients of all ages, from toddlers to seniors.
His commitment to excellence is demonstrated through his extensive continuing education, including advanced training at The Pankey Institute, Invisalign certification, dental implantology coursework, and active membership in the Spear Education Study Club. Dr. Koshki is dedicated to creating beautiful, healthy smiles in a gentle, compassionate environment, supported by his carefully assembled team who share his commitment to exceptional patient care.
Gail Sakamoto, D.D.S.
Dr. Gail Sakamoto joined Santa Monica Dental Arts by Dr. Moeiz Koshki in January 2019. She brings with her over 25 years of experience. A graduate of UCLA Dental School, she works hard to bring her patients the best dental care. She is very empathetic and caring to patient’s feelings and is very meticulous. She also tries to preserve as much of the patients’ teeth as possible, earning the title of Dr. SuperTooth Hero.
When not in the office, Dr. Sakamoto enjoys spending her spare time with family and friends; having dinners, drinking wine, and baking. The new love-of-her-life is her adorable grandson, whom she dotes on. She likes going to Napa and the Bay area to visit her son, and she hopes to do more traveling in the future; seeing the US, Japan, and Europe.
David Koshki, DDS
Dr. Moeiz Koshki is proud to announce that his son, Dr. David Koshki, has joined his practice in Santa Monica, providing considerate care to all of our patients. Dr. David Koshki is a proud graduate of the USC Herman Ostrow School of Dentistry, where he completed both dental school and dental hygiene school. With a strong passion for cosmetic dentistry, Dr. David specializes in enhancing the smiles and confidence of their patients, creating natural and beautiful results tailored to each individual.
Dr. David is committed to treating every patient like family, taking the time to understand their needs and ensuring they feel comfortable and cared for during every visit. With a focus on providing high-quality, personalized care, Dr. David strives to create a positive, stress-free dental experience for all.
Outside of dentistry, Dr. David enjoys staying active, whether it’s playing basketball and soccer or spending time at the beach and exploring the outdoors.
